Please complete the form below so we can be prepared to receive your artwork, and return it to you properly.
If shipping your artwork in a non-commercial shipping container, there is a $25 fee for uncrating (payable online via PayPal upon completion of this form).
If you will be hand-delivering your artwork, or sending your artwork in a commercial art box, such as AirFloat, MasterPak, Uline, etc., you do not need to pay the un-crating fee. However, you still need to complete the form below. NOTE: Federal Express Premium Art Boxes are acceptable but the Federal Express large frame box is not. Also, Home Depot boxes and homemade art boxes do not qualify under the un-crating fee exemption.

Prepare Your Pre-Paid Return Shipping Label
OPA requires that you include a pre-paid shipping label to cover the return shipment of your painting if it is not sold. And, please do not forget to insure your painting!
There are two ways to send your label:
1. Include a paid, printed return shipping label with a bar code and send it in with your painting. Be sure to use the yellow envelope to make it easy for the museum to see your label.
2. Email your pre-paid shipping label directly to California Center for the Arts, Escondido Museum at bmarino@escondido.org. Be sure when you email your label that you include your name in the subject line. Example: “John Michael Carter – Return Shipping Label”.
